Program Overview

ICAN’s Adult Care Management services are vital to the lives of the individuals that we serve. Our Care Coordinators work in a person-centered approach to help our clients become healthy and successful on their own terms. Our Care Coordinator/Client relationships offer the encouragement to help individuals achieve goals they have set for themselves.

Through Adult Care Management, we strive to improve the physical, mental and emotional well-being of adults facing a serious mental illness, HIV/AIDS and/or two or more chronic medical conditions.

Criteria

  • Must be an adult aged 18 and older.

  • Must be diagnosed with a serious mental illness, HIV/AIDS and/or two or more qualifying chronic medical conditions.

  • Individuals must have active Medicaid.

Making A Referral

Referrals for Care Management services can be made by contacting our Outreach Manager, Margaret Willson at (315) 731-2611 or Outreach Specialist, Jennifer Evola at (315) 731-2666.

Referrals can be made over the phone or in person. Any individual who feels they’re in need of services may self-refer or have a family member, friend or other service provider complete the referral on their behalf. All of our referrals are faxed in and filtered through Central New York Health Home Network, and are then assigned to us, the Care Management agency, for outreach and enrollment.

We assist them with succeeding in any goal they may have set, whether it be recovering from addiction, finding success in the classroom, creating a healthy home for their family as they anticipate the birth of a child, or seeking support and community resources, all while keeping families together as they process through their traumas.

Access To Resources

We connect individuals and families with critical community resources, such as access to physical and mental health providers, employment opportunities, housing options, peer support groups, and Wraparound services. We understand that access to some of these services can be difficult to attain during certain life challenges, and we are here to help individuals and families overcome this obstacle.
