    Integrated Community Alternatives Network (ICAN), a Community-based Wraparound provider of Social and Mental Health services with an emphasis on treating the client in a family context, is currently seeking innovative and experienced applicants to fill a Full-Time Clinical Care Coordinator position for Herkimer County Community College. 

    Clinical Care Coordinator (LMHC/LMSW/LCSW) will work collaboratively with college faculty and staff, ICAN team members, and community providers to deliver trauma-informed, strengths-based, and solution-focused services. The ideal candidate is passionate about supporting young adults through the unique challenges of college life while promoting independence, wellness, and personal growth.

    Annual Salary: $60,000-$65,000, Non-Exempt, paid at an hourly rate.
    35-hour workweek, Monday - Friday

    • Provide brief therapeutic support and solution-focused interventions for students experiencing emotional, behavioral, or situational concerns.
    • Conduct triage assessments to determine student needs and connect them with the appropriate level of care.
    • Provide crisis intervention and de-escalation services as needed.
    • Assist with front office coverage during emergencies or high-need times.
    • Support students who walk in with referrals and help connect them to the appropriate services.
    • Provide consultation and backup support to staff as needed.
    • Meet with students who self-identify as needing support, including general check-ins, life coaching, time management, and wellness support.
    • Conduct brief intake conversations to assess student needs and connect them with appropriate campus and community resources.
    • Assist with or help coordinate virtual workshops as needed.
    • Support departmental events and student engagement activities.
    • Participate in pop-up tables and outreach efforts to increase student awareness of available services.
    • Complete student intake documentation.
    • Coordinate referrals and follow up with students to ensure continuity of care.
    • Collaborate with campus and community partners to address student support needs.
    • Assist with resource development projects.
    • Ensure student documentation is completed promptly following each student interaction.
    • Current New York State licensure as one of the following: Licensed Mental Health Counselor (LMHC) Licensed Master Social Worker (LMSW) Licensed Clinical Social Worker (LCSW)
    • Master's degree in Counseling, Social Work, Mental Health Counseling, or a related behavioral health field.
