Program Overview

The Home-Based Crisis Intervention Service prevents psychiatric hospitalizations or out-of-home placement. HBCI services are flexible to meet the individual needs of the youth and families we serve. 

HBCI Services include:

  • Home-based Assessment & Treatment Planning
  • Conflict Resolution
  • Life Skills
  • Safety Planning
  • Psychoeducation
  • Family & Individual Therapy
  • Intensive Case Management
  • Service Linkage

For support, HBCI staff are available to families 24 hours a day, 7 days a week.

Criteria

  • Youth must be at risk of inpatient hospitalization or out-of-home placement.

  • The identified child and family in crisis are seeking an alternative to psychiatric hospitalization or out-of-home placement.

  • The young person is between the ages of 5 and 20 years, 11 months old, and lives in Oneida County.

Making A Referral

HBCI accepts referrals from parents/caregivers, community members, schools, children's mental health providers, other agencies working with children, and psychiatric hospital emergency departments. To make a referral, contact Carrie Conte, LCSW at cconte@ican.family or (315) 941-4889.

Access To Resources

We connect individuals and families with critical community resources, such as access to physical and mental health providers, employment opportunities, housing options, peer support groups, and Wraparound services. We understand that access to some of these services can be difficult to attain during certain life challenges, and we are here to help individuals and families overcome this obstacle.
