    Kids Herkimer

    Integrated Community Alternatives Network (ICAN), a Community-based Wraparound provider of Social and Mental Health services with emphasis on treating the client in a family context, is currently seeking innovative and experienced applicants to fill a Full-Time Family Service Coordinator position in the Kids Herkimer program.

    The Family Service Coordinator works collaboratively with Herkimer County DSS, Family Court, schools, behavioral health providers, and other community partners to reduce the need for out-of-home placement while helping families achieve lasting stability and positive outcomes.

    Duties and Responsibilities include the following.

    The annual salary generally ranges between $45,000-$49,000. This range is an estimate based on an applicant’s skills and experience.

    This is a non-exempt position and paid at an hourly rate. 35-hour workweek, Monday - Friday

    • Conducts outreach to engage child(ren) and assist families with an understanding of agency philosophy and policies
    • Maintain a caseload of up to 12 families, providing strength-based, family-centered, and culturally competent care and quality service to each family.
    • Support permanency planning efforts by working collaboratively with families, agency personnel and Herkimer County DSS to prevent out-of-home placements.
    • Develop, implement, and monitor individualized service plans/plans of care in collaboration with families, supervisor, and Herkimer County DSS.
    • Conduct a minimum of two face-to-face contacts per month with each child and family on the caseload, including home visits.
    • Link clients and families to appropriate resources and services in a timely, coordinated manner. Maintain routine follow-up on linkages and referrals made.
    • Advocate on behalf of children and families to reduce barriers to services and promote access to community resources that support safety, permanency, and well-being.
    • Schedule and facilitate family team meetings, case conferences, and required quarterly service plan review meetings with families, service providers, and other relevant stakeholders to evaluate progress, revise service plans, coordinate supports, and ensure services remain responsive to the family's strengths and needs.
    • Serve as a liaison between client/family, Herkimer County DSS, agency personnel, family court, and community service providers as needed.
    • Identify and utilize formal and informal community supports and networks to meet client/family needs.
    • Attend all required meetings, case conferences, Family Court appearances, and other program-related activities as assigned.
    • Maintain accurate, timely, and comprehensive client documentation and case records, including progress notes, service plans, service summaries, court reports, and case updates, in accordance with ICAN, Herkimer County DSS, and all applicable state and federal regulations. Prepare documentation for Family Court and other authorized stakeholders as required.
    • Report incidents promptly and engage in quality improvement initiatives as directed by the supervisor.
    • Travel within Herkimer county to meet clients in their homes, schools, or other relevant settings.
    • Trainings as necessary and scheduled.
    • Bachelor's Degree in Human Services or related field with at least one year of experience working with youth and their families.
    • Experience working in child welfare setting preferred.
